Public Health Nurses are required by law to submit the Public Health Services
Report Form and Report of Contact Form when there is a suspected SIDS
death.

Coroners are required by law to report the information pertaining to a
SIDS death within 24 hours after the gross autopsy. The coroner should
report the death to the health department of residence. The coroner should
also complete a Coroner Notification Card DHS 4411 (8/02) and send it
to the California SIDS Program. For more information or to request forms,
