Registration Information
Please use the Conference Registration Form to register for the conference and all pre-conference events. The form can be downloaded as an attachment, completed and returned via email. Another option is to complete the form and return it via fax or mail. Call the California SIDS Program to register via phone at 1-800-369-SIDS (7437) within California or 916-851-SIDS (7437) outside of the State.
To register, a completed Conference Registration Form and full payment of fees by personal or institutional check is required and must be postmarked by Friday, September 17, 2010.
Cancellations must be requested in writing and received by Friday September 24, 2010 for a refund. No requests for refunds will be accepted after this date; however a substitution of the registrant will be permitted at the time of cancellation.
Please note we are NOT able to accept credit cards or agency purchase orders. Payment may be made by cash, money order, cashier's check, personal, business and/or institutional checks
On-site registration will NOT be possible. Registration is required for all pre-conference events and conference activities to ensure all attendees can be accommodated.
SIDS Coordinators and public health professionals are encouraged to register and attend this all day educational event. Local health jurisdictions can utilize MCAH SIDS funds to support conference costs including travel and registration fees.
How to Register
You may register for the October 5, 2010 Annual California SIDS Program Fall Conference, Angels Among Us and all pre-conference events scheduled for October 4th by email, fax, mail or by phone. Each person attending must complete a Conference Registration Form. Download additional forms by accessing the California SIDS Program website at www.californiasids.com or contact the California SIDS Program at 916-851-SIDS (7437) or 800-369-SIDS (7437) within California or email us at info@californiasids.com for more information.
Healthcare professionals and public health nurses are encouraged to submit the Conference Registration Form while awaiting payment of registration and/or CEU fees by your agency.
Registration/CEU Fees
Registration for all pre-conference events being held on Monday October 4, 2010 is requested. There are no fees to attend the Monday afternoon workshop, SIDS: Making Memories, or any of the other events including the memorial service or welcome reception. We encourage you to pre-register for these events so that the sponsoring organization, The Guild for Infant Survival of Orange County can plan for and accommodate all those wanting to participate. You are reminded that anyone can attend Monday's complimentary activities. It is not necessary to enroll in the educational Conference session on Tuesday October 5th to enjoy the pre-conference events. Complete the requested information on the Conference Registration Form or call the California SIDS Program to pre-register for any or all of the events scheduled for Monday, October 4th.
Registration fees for Tuesday, October 5, 2010 include: admission to the conference; a continental breakfast; conference materials; speaker handouts; and entrance to displays and exhibits. The Conference Registration Form along with payment of fees must be postmarked by Friday, September 17, 2010.

Registration confirmation notices will be sent via email, fax or mail along with a directional map to the Doubletree Hotel Anaheim/Orange County, site of the conference, upon receipt of a completed registration form and payment of fees.
CEU Credits/Certificates
The California SIDS Program is approved as a provider for continuing education by the California State Board of Registered Nursing, Provider Number 12781. The California SIDS Program has approved this course for six (6) contact hours.
All participants requesting CEU contact hours must complete the appropriate information on the Conference Registration Form. An additional fee of $25 will be required. Please refer to the Registration/CEU Fees for details.
The California SIDS Program in partnership with the California Department of Public Health, Maternal, Child and Adolescent Health Division will provide certificates of training to all other participants at no cost.
Financial Assistance
Financial assistance to attend the conference is available for SIDS parents, family members and others affected by the sudden unexpected death of an infant. Limited funds are available through several of California's SIDS Parent Support Organizations to help cover registration fees, hotel costs, and/or parking fees.
If you would like to attend the conference, reside in California, and need financial help, please contact the California SIDS Program as soon as possible via email at info@californiasids.com or call our toll free number 1-800-369-SIDS (7437). We will provide you with the information to apply for assistance. Due to the limited availability of funding, financial assistance will be on a first come basis.
You should also complete the Conference Registration Form and forward it to the California SIDS Program. Do not send your registration fees if you are requesting financial assistance. Please submit your form as soon as possible due to the limited funding available.
Dress
Dress attire is casual. The conference facilities are air conditioned. A sweater or light jacket may be needed. Comfortable shoes are recommended for those walking to/from the hotel and nearby eating establishments.
You may want to bring some warm clothing and tennis shoes for early morning/evening activities.
Food
A continental breakfast of coffee/tea/juice, fresh fruit and bakery goods will be provided on Tuesday, October 5th from 8:00 to 8:30 am. The continental breakfast is included in the registration fee.
Lunch will not be provided. There are a variety of cafes, restaurants and fast food vendors within walking distance of the nearby The Block shopping center. The Doubletree Hotel has a full service restaurant on site as well as a Starbucks.
Some hotel guest rooms have a refrigerator, microwave and coffee maker with complimentary coffee and tea. Hotel room service is available upon request.
