| This four (4) hour morning course complies with SB 1068 which requires SIDS education be provided to all persons who interact with parents and caregivers following a SIDS death. |
| Public Health Professionals |
This day long course meets the requirements of SB 362 to prepare public health professionals with basic counseling skills and knowledge about SIDS to support families and childcare providers impacted by this tragedy.

Special Guest Presenter: Thomas G. Keens, MD Professor of Pediatrics, Physiology and Biophysics, Keck School of Medicine of the University of Southern California Division of Pediatric Pulmonology, Children's Hospital, Los Angeles

Dr. Keens will present an overview of SIDS theories and current research findings. He will discuss the newly expanded 2011 American Academy of Pediatrics SIDS risk reduction and safe sleep recommendations, explore the issue of unsafe sleeping environments and outline educational interventions to protect babies from suffocation, SIDS and other sleep-related infant deaths.

Emergency Responders - EMTs and Paramedics
- This course has been approved for four (4) hours of continuing education by an approved California EMS CE Provider and is instructor based. California EMS CE Provider #34-3100.
- There is no cost for continuing education units or training materials.
- To request CEUs, please check the appropriate box on the training registration form and provide your EMT/paramedic license number.

Public Health Professionals - RNs and PHNs
- Six and one-half (6.5) CEUs are offered for RNs. Provider approved by California Board of Registered Nursing, Provider Number 12781.
- Public health professionals requesting CEUs must attend all day. There are no credits for partial attendance.
- There is no cost for continuing education units or training materials.
- To request CEUs, please check the appropriate box on the training registration form and provide your RN license number.

| Registration/Training Information: |
- Pre-registration is required as space is limited. Register by phone, fax or online (email).
- The training announcement and registration form are accessible by clicking on the links below.
- Registration will close at the end of business on Wednesday, May 30, 2012.
- There is no registration fee for this training.
- Attendees will receive a packet of informational/educational resources and printed speaker power point presentations.
- Upon receipt of a completed registration form, the California SIDS Program will send enrollees a directional map and registration confirmation notice.

| Security Requirements: |
- Onsite registration is not available as all attendees must be on a visitor's list which the California SIDS Program must submit to CDPH prior to the training date.
- Attendees must present to the security staff your personal photo identification such as a state-issued driver's license, state issued identification card, passport, etc. on the day of the training.
- You must sign in and out at the security desk located in Building F (adjacent to Parking Lot # 3) for entry/exit into the Auditorium where the training will be held.
- Parking is available only in the Visitor Lots in front of Building F or Building P. If the Visitor Lot is full, the security guard at the main entry gate will direct you to a different parking lot. Do not park along the curbs. (Note: Registrants are encouraged to car pool as space is limited.)
- You will be required to pass through the metal detector and your carry-in items will be scanned using an x-ray machine. (Please allow extra time the morning of the training for this procedure.)
- You will be issued and must wear a visitor's sticker.
- Visitors who do not cooperate with the security polices of CDPH will be asked to leave the premises.

| Food: |
- A continental breakfast will be provided from 8:30 am to 9:00 am. Attendees are encouraged to arrive early and allow extra time to get through the security check-in, sign-in/receive training materials from the California SIDS Program staff and enjoy morning refreshments. The training will begin promptly at 9:00 am.
- Lunch is not provided. The California Department of Public Health Richmond Campus has a cafeteria on-site which is one option for lunch. For planning purposes, attendees wanting to have lunch at the Gourmet Choice Cafeteria are requested to indicate this on the training registration form. There are also fast food restaurants/eating establishments nearby. Enrollees may bring their lunch as the training room will remain open during the lunch break.

| Transportation/Shuttle Service: |
- One transportation option is BART. Those attendees using BART should exit at the El Cerrito Del Norte station. Exit the BART station on the uncovered parking lot side. Look for the shuttle bus sign with DHS or CDPH indicated. Refer to the CDPH Richmond Campus Shuttle Service schedule for arrival/departure times to and from the facility's main entrance location.
- Registrants interested in using the CDPH shuttle service must advise the California SIDS Program upon receipt of your registration confirmation notice. Your name will then be provided to CDPH so that you will have access to this special free shuttle service. Only those who have signed up will be permitted on the BART shuttle to/from the CDPH Richmond Campus.

| Travel/Hotel Information: |
- There are several hotels less than 7 miles/10 minutes from the training location. Two hotels suggested by CDPH are in the Richmond/Berkeley area.
